Commissioning Service Request

The City of Westmount offers a swearing-in service enabling residents to have certain documents signed in the presence of a commissioner for oaths. This service is available by appointment only at the town hall.

Before submitting your request, please ensure that your document is intended for use in Quebec and that it can be sworn by a notary public. This requirement does not apply to life certificates.

Please note that the City of Westmount does not offer a document certification service.

At your appointment, you will need to:

  • Present two valid forms of identification, one of which must be photo ID;
  • Bring the original document to be sworn;
  • Refrain from signing the document before the appointment, as the signature must be provided in the presence of the commissioner;
  • Pay the fee of $5 per signature.

Please complete the form below to submit your application. Once your application has been approved, you will receive a link enabling you to book an appointment with us.

Please note: If you are 15 minutes or more late, your appointment will be cancelled.
