A maximum of seven cards per family. A $5 fee for each additional card. All persons 18 years of age and older must provide two (2) recent (last 3 months) proofs of residency and one (1) piece of photo identification.

Children : Anyone between the ages of 4 and 17 is required to provide one (1) proof of residency along with one (1) proof of age.
